Cleanup the IDL structures for subscriptions, publications, etc. (in the EC).

[STATUS] Part of this was completed. The Header and Payload of the events are
clearly distinguished, now we need to use only the Header in the Publication and
subscription definitions.

This is a valid enhacement request, but fixing it may require breaking the
current interfaces; we need to find an appropiate solution that will satisfy our
sponsors.

I'm upgrading the priority, it has become more of a problem with the new
filtering primitives added into the RTEC.

With the addition of the prefix filter builder this problem has become less
pressing.  It is still an interesting problem, but nothing to loose sleep over.

I will never have time to work on these bugs.  Returning them to the TAO support
pool.
