Reminder bug in case I don't get to this soon and forget it. The KAI/KCC 
compiler has been bought by Intel in 2000 and we are going to remove all 
support for it from ACE/TAO.

          Removed these files which are for the KAI C++ compiler. This
          compiler doesn't exist anymore and full support for KAI C++
          has been removed now from ACE/TAO. See bugzilla bug 2007
