We have a RW connection handler that works well for Synchronous cases. This 
helps single-threaded pure clients to optimize stuff.  It would be great to see 
something similar for AMI cases. I dont have much ideas now, but will include 
my thoughts after x.2 goes out.

Whenever this gets addressed, we should make sure we handle DII send-deferred 
as well.

RW with AMI will work with the upcoming TAO 2.1 release
